Do IUDs Cause Pelvic Inflammatory Disease?

IUDs are considered one of the most effective methods of birth control, but they are not without risk. Though opinions vary, some women choose to forgo IUD use because they are concerned about potential side effects. A common concern associated with IUD use is pelvic inflammatory disease (PID). Fortunately, there is no medical proof that using an IUD results in an increased risk for developing PID.

What is PID?

PID is an infection that occurs in the pelvic organs. This includes the fallopian tubes, the uterus, and the cervix. It is the result of a sexually transmitted disease, typically Chlamydia or gonorrhea. In many cases there are no symptoms of PID, but women may experience abdominal pain, a fever, painful urination, or vaginal discharge. Intercourse might also be painful. PID is treatable with antibiotics, but since there are sometimes no symptoms it can go untreated for long periods of time. This can lead to scarring and infertility.

Can IUDs Cause PID?

Research has shown the risk of PID is not increased by the use of an IUD, but health professionals believe there is likely some kind of connection. Most agree this is due to the bacteria that may be present during the insertion of the IUD. The bacteria can cause an infection in the pelvic organs which leads to the development of PID. If PID develops within the first month of IUD infection, there is likely a link to the insertion process. However, this risk all but disappears after a month of use.

Some research has shown that hormonal IUDs may actually decrease the risk of PID. The progestin released by this type of IUD creates a thickening of the cervical mucus and causes endometrial changes. These conditions likely lead to a lowered risk of developing an infection. This benefit does not appear to be present with copper IUDs.

Regardless of the science, you will likely come across some anecdotal evidence that a woman’s PID was caused by her IUD. If you are concerned about this happening, speak with your doctor. He or she can assure you that the connection is related to the insertion process, so if you can avoid an infection during this time, there should be no problems. Be sure your doctor has experience with inserting IUDs and ask for details about what is done to keep the process sterile.
