The ParaGard is a deceptively simple device: a small plastic T with copper coiled around it that’s inserted into the uterus. Once it’s there, the copper and presence of a foreign body make the uterus a hostile environment for sperm, preventing the fertilization of an egg. Because it works without releasing hormones or medication and doesn’t require interrupting sex to reach for contraception, there are few unpleasant side effects for most users. But every medical device has potential side effects, and Paragard is no exception.

Some side effects of Paragard are relatively minor. Once inserted, it can make period bleeding heavier and cramping worse. According to one source, “Heavier menstrual flow with copper IUDs might be caused by vascular changes, which regulate blood flow to the uterus (7,9). In studies, these blood flow changes were found to be greater in people using a copper IUD with heavy menstrual periods compared with copper-IUD users with normal menstrual bleeding.” This means the copper IUD may be a less than ideal solution for those whose periods are already very heavy. For others, a slight increase in periods might be tolerable or not even noticeable.
Anemia is a side effect of heavy bleeding. It’s a shortage of iron that can result when blood is lost through menstruation, and can result in feelings of weakness or tiredness. Anemia can be addressed by regularly taking an iron supplement or eating iron-rich foods.

The IUD can both worsen existing menstrual cramps and cause cramps at other times. For those who experience severe cramps that don’t fade with time, it may be necessary to have the IUD removed and try another birth control method.
More serious results occur when the IUD embeds in the uterus, either because it is improperly inserted or because it simply wanders from its correct location. If this occurs to a minor extent, it may simply be difficult to remove when the patient is ready. If it’s extreme, it can cause severe bleeding, abdominal pain, fever and other symptoms. These may worsen if the IUD makes its way out of the uterus altogether and begins to interfere with other organs.

Another rare effect of IUDs is pelvic infection. In rare cases inserting an IUD can transmit germs up through the vagina into the uterus, where they cause fever, pain, and potential damage to the tissue such as abscess and scarring. This is more likely to occur when the patient has an active chlamydia or gonorrhea infection—rare, since doctors typically test for these infections and ensure they are cleared up before inserting an IUD.
Although these bad outcomes are very rare, they have been the subject of lawsuits from women who have endured surgery and had long-term effects on their fertility from their IUD; all patients should be aware of the potential outcomes of the birth control they choose.
