Patients who are considering a Paragard IUD for the first time have many questions. This small plastic and metal device works very differently than other forms of contraception, and has a different set of benefits and potential downsides.
What is the ParaGard?
The ParaGard is a t-shaped device made of plastic wrapped with copper, about 1.5” long. It is inserted through the cervix into the vagina through a small plastic tube, and at that point its arms extend outward and keep it in place.
How does it work?
The effect of a foreign body in the uterus, combined with the changes in the uterine environment caused by copper, prevent sperm from fertilizing an egg. It’s over 99% effective. The Paragard releases no hormones and can remain in place for up to 10 years.

Can I feel the ParaGard?
In most cases, no. The ParaGard can cause an increase in menstrual cramps but is otherwise not perceptible.
Can my partner feel it?
It doesn’t extend into the vagina, except for a pair of strings that help a doctor pull it out when it’s time for removal. The strings typically wrap around the cervix and can’t be felt. If the patient’s partner complains about being able to feel the strings, a gynecologist can trim them or reposition them.
Is the ParaGard reversible?
It’s 100% reversible, and users can begin trying to get pregnant almost as soon as it’s removed.

What is insertion like?
Paragard is a brief outpatient procedure that takes place in a gynecologist’s office. The doctor will dilate the cervix, flush the vagina and uterus out with saline to prevent infection and insert the device. The process of inserting an object through the cervix causes discomfort, so patients are advised to take a painkiller beforehand.
How soon does it start working?
Right away! It can even be used as emergency contraception if inserted within 5 days after unprotected sex
Who should and should not use the ParaGard?
It is recommended for sexually active people seeking a highly effective, reversible contraception. Since it doesn’t prevent STI transmission, users should be in a monogamous relationship or willing to use backup birth control methods.

It’s not recommended for those who want a method that’s immediately reversible (since taking it out requires an office visit). ParaGard can’t be inserted if the patient has an active case of gonorrhea or chlamydia; the STI must be treated first. What are ParaGard’s side effects?
Paragard can increase menstrual bleeding and cramping, especially in those who already have heavy periods. More rarely, it can cause serious health problems. IUD insertion increases your chances of infection within the uterus, although this is rare because healthcare providers take steps to make the inserting process sterile.
ParaGard can sometimes begin to embed in the uterine tissue. This can range in seriousness; it can cause no symptoms but make removal difficult, or if the ParaGard continues traveling through the uterus, it can perforate the organ entirely and begin interfering with other organs. This can cause long-term effects to fertility and necessitate surgery to remove the device and repair the damage.
Compared to other forms of contraception, Paragard can offer fewer side effects and a lack of interruptions during sex. But those considering it should be aware of all the potential outcomes, including rare risks.
